P0141 is on

Hi Newbie here, Getting a Check engine light every so often. PO141. does this mean I have to replace the sensor, or is it sniffing something it doesn't like? have a 2000 Montana V6.
 
Most likely the downstream O2 sensor, bank 1. Bank 1 is the side with cylinder #1, and the downstream O2 sensor is the one after the catalytic converter. I'm not sure if that year Montanna has 2 cats or only 1. If it has only 1, then the only possible choice would be the downstream sensor. I've replaced many O2 sensors for this code, and rarely find this code set from a wiring issue, but it is a good idea to confirm nothing is wrong with it. Like a rubbed thru wire somewhere.

If the error shows P0135 (Bank 1, Sensor 1). The sensor is located on the exhaust manifold.
If the error shows P0141 (Bank 1, Sensor 2). The sensor is located next to the catalyst. (See picture)
