Rough idle / whirring when accelerating

Very rough idle on cold start (not as bad if the car is started again after the engine has warmed up), and a very loud whirring noise when accelerating (low RPM 1-3k).

I've previously had a P0171 code which was always intermittent and hasn't reappeared for a few months, and when I took it to a mechanic they said it was a faulty EGR valve which they can't replace because the PCM needs to be reprogrammed by a dealer after the installation.

I'm just basically hoping to get a second opinion.

I would find another independant shop that is equipped to replace the EGR.
On the noise take a length of rubber hose, hold one end against your ear and move the other end around the engine compartment to determine the source of the noise.
