Can't remember for sure but it's under the dash between the glove box and firewall. I think you have to unlatch the glove box then push the release tab(s?) to swing the door out all the way. Then it's just several small bolts around the circumference of the housing. Just lay on your back on the passenger side and look under the dash. You'll see it. Also hook the motor up directly to a 12v source to make sure the motor itself is bad. Usually it's the resistor/relay (can't remember what its called). It's located right there beside the motor and sticks up through the housing.
