Pontiac Grand am Hesitates when i accelerate and idles rough, Whats wrong?

my car hesitates really bad when i accelerate and idles really rough when i come to a stop. It even stalls out from time to time. I have used fuel injector cleaner, changed the spark plugs and changed the TPS. Any ideas what could get me up and going again?

is your check engine light on? did you check your fuel pressure?
 
Did you fix this yet? Check the MAP sensor vacuum hose. It is probably rotting away.
